Yorkshire Nerve Clinic is the private practice of Dr Shaun E.C. Foo, Consultant Neurologist and Clinical Neurophysiologist. The data controller is Brainwave Medical Ltd (company number 14326150), trading as Yorkshire Nerve Clinic. You can contact us about anything in this policy at admin@yorkshirenerveclinic.com.
Depending on how you use this website, we may collect:

Enquiry form: your name, email address and phone number; if you are enquiring as a patient, your date of birth, preferred clinic, funding or insurer, a brief description of your enquiry, and any referral letter you choose to upload; if you are enquiring as a clinician or organisation, your role and organisation and your message.

Chat: any messages you send through the website chat, and the contact details you provide in them.

Website usage: technical data such as your device, browser and pages visited, collected through cookies by our website platform. You can manage cookies through the cookie banner, and more detail is in our cookie policy.
Please do not include detailed medical information in the enquiry form or chat; clinical information is best discussed at your appointment.
We use this information to respond to your enquiry, to arrange appointments, and to administer the practice. Under UK data protection law, our lawful bases are: taking steps at your request before entering into a contract of care (Article 6(1)(b)); our legitimate interests in running the practice and responding to enquiries (Article 6(1)(f)); and compliance with our legal obligations (Article 6(1)(c)). Where your enquiry includes information about your health — for example a referral letter — we process it for the provision of healthcare under Article 9(2)(h).
Who we share it with. Only as needed to arrange and provide your care: the bookings team at the hospital where you wish to be seen; your insurer, where you are using private medical insurance; your GP and/or referring clinician, in line with normal clinical correspondence; and our practice administration e.g. Dr Foo's medical secretary. Our website and its forms are provided by Wix.com Ltd, which hosts this data as our processor; Wix may store data outside the UK, and does so under safeguards recognised by UK law (including standard contractual clauses). We do not sell or share your information for marketing.
How long we keep it. Website enquiries and chat messages are kept only as long as needed to deal with them and are then deleted after your appointment is complete. If you become a patient, information forming part of your clinical record is kept in line with national guidance on the retention of medical records.
Your rights. You have the right to ask for a copy of the information we hold about you, to have it corrected, and — in some circumstances — to have it deleted, to restrict or object to its use, or to receive it in a portable format. To exercise any of these rights, contact admin@yorkshirenerveclinic.com. You also have the right to complain to the Information Commissioner's Office (ico.org.uk).
Changes. We may update this policy from time to time; the current version will always be on this page, with the date it was last updated shown below.
